We are seeking a part time compassionate and committed experienced Advanced Practice Nurse to work with Stein Hospice and Stein Assisted Living on our HOSPICE AND PALLIATIVE care service lines.
(Advanced Certified Hospice & Palliative Nurse Required) We want to hear from you if you have the following experience in hospice and palliative care:
Collaborate with the patient's care team - internist, oncologist, radiation oncologist, surgeon, advance practice providers and, other health care team members to develop practical care expectations based on goals of care.
Facilitate patient and family decision regarding goals of care and perform advance care planning visits.
Prescribe or recommend drugs or other forms of interventions.
Monitor patient's response to treatment modalities and modify plans as indicated.
Assist in all facets of care coordination for palliative care referrals.
Communicate with patients and families to improve their knowledge of diagnosis, prognosis, and treatment plans.
Provide in service for team members regarding palliative care, serious illness, and end-of-life care management.
Partner with palliative care program director in the development and growth of the Palliative Care Program.
Document and bill accurately for palliative care services provided.
Must be able to travel to multiple practice locations within Somerset, Middlesex, Mercer, Hunterdon and Union Counties.
Minimum 1 year experience in hospice, palliative and long-term care setting ACHPN required Immediate need for this part time position with flexible hours.
MUST BE COVID VACCINATED AND BOOSTED.
