has been providing community-based services to people with intellectual and developmental disabilities for more than 40 years.

Summary Within our Clinical Department, Everas Social Workers provide case management to clients in our Residential and Day programs to ensure they receive supports and services to lead productive and enriched lives in the community.
The successful candidate will be self-motivated, possess strong case management skills and have a strong ability to prioritize and problem solve.
Your commitment to excellence in disability services, collaborative mindset, professionalism, and high-level organizational skills are needed to achieve the best possible outcomes for the people in our programs.
Social Worker duties include but are not limited to:
Conduct frequent site visits to client's homes and vocational programs to ensure appropriate identification of goals, service planning, and implementation of services Participate in the Admission and Client Transition Committees Complete and/or coordinate client assessments and evaluations Identify needs, advocate for clients, and coordinate services to ensure clients receive necessary services, supports, and resources Participate in the development and delivery of Individual Service Plans Ensure clients secure financial benefits, specialized equipment, and other community resources.
Act as a resource to staff providing direct services Assist with admission, transfers, and discharge planning Promote collaboration and interdepartmental cooperation Participate in specialized training and projects as requested Recommend policy and procedures to department to improve service delivery What we offer:

Master's degree in Social Work, MSW Experience with intellectual and developmental disabilities preferred Detail oriented with strong written and verbal communication skills Flexible schedule to include evenings/weekends Criminal Background Check Valid NJ Driver's license Willingness to meet NJ Executive Order 252, which requires employees to be fully vaccinated for COVID-19, including a booster; at minimum one dose is required to begin employment with the expectation to complete subsequent doses as per recommended time frames; this requirement may be waived only in exceptional circumstances as outlined in the law.
